REQUEST A FIT NOTE

Before requesting a fit note, you can self certify for 7 days. For more infomation about this ask your employer.

To request a sicknote do this via eConsult.  https://bewickemedicalcentre.webgp.com/

For new requests you may need to see a GP, our reception team will contact you once you have submitted your eConsultation.

Please allow 7 working days for your fit note to be completed. Your fit note will be back dated.

We generally sent them to your email address but you can request a paper copy.
